1-2 years acute care experience, 1-2 years home care experience, Bachelor's degree, 1 year hospice care experience
What You'll Do.
Complete patient and family assessment
Provide physical assessment and history
Provide professional nursing care
Write and initiate plan of care
Re-evaluate patient and family needs
Revise plan of care as necessary
Develop care plan with goals
Include patient and family in planning
Initiate preventive and rehabilitative procedures
Administer medications and treatments
Counsel patient and family
Provide health care instructions
Assist patient with activities of daily living
Coordinate patient care
Maintain clinical notes
Record pain/symptom management changes
Communicate with physician
Obtain physician orders
Communicate with community health persons
Teach patient and family self-care
Provide medication and diet instructions
Provide safe environment for patient
Assist family in continuity of care
Meet emotional needs of patient
Attend interdisciplinary group meetings
Participate in on-call duties
Ensure equipment and services are available
Supervise ancillary personnel
Delegate responsibilities
Maintain and upgrade professional knowledge

We are centered on caring for the whole person – with expert medical care, pain management as well as emotional and spiritual support along with resources, information, and emotional support for families and caregivers. About the role: The Registered Nurse Case Manager plans, organizes, and directs hospice care and is experienced in nursing, with an emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities. Job Responsibilities: Patient Care Completes an initial, comprehensive, and ongoing comprehensive assessment of the patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). Provides professional nursing care by utilizing all elements of the nursing process. Assesses and evaluates patient’s status by: Writing and initiating a plan of care Regularly re-evaluating patient and family/caregiver needs Participating in revising the plan of care as necessary Initiates the plan of care and makes necessary revisions as patient status and needs change. Uses health assessment data to determine nursing diagnosis. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions.
