Member in good standing with the College of Nurses of Ontario, Successful completion of Basic Cardiac Life Support (C level) Heart Saver Plus (Canada) required, 2-5 years experience in the care and management of patients with moderate to advanced dementia, 2-5 years experience in care and management of patients with moderate to advanced behaviours and psychological symptoms of dementia, 3- 5 years experience in care of medically complex geriatric patients, Computer proficiency required, Good Attendance required

Nice to Have

Baccalaureate degree in Nursing preffered, Experience in Geriatric Psychiatry, Geriatric Medicine, or Geriatric Rehabilitation preferred, Certification in gerontology preferred, Member, Registered Nurses Association of Ontario (RNAO), OARN (SIG) preferred

What You'll Do.

evaluate and coordinate comprehensive professional nursing care for patients and families within the context of an inter-professional practice and primary nursing model

Use theoretical and empirical knowledge and the input of team members to plan and implement care for patients with dementia and their families

Education of patients

referring facilities and other community providers regarding care of older adults with dementia

Participate in inter-professional team care planning forums

inter-professional rounds and family conferences

supervision and mentorship to students and new staff on the unit related to Geriatric Rehabilitation nursing

Participate in professional

program and hospital-wide initiatives

including professional development of self

Compliance with and commitment to patient safety responsibilities and corresponding hospital plans

policies and procedures in order to ensure a safe environment for patients
