What You'll Do.
Manage Home Hemodialysis (HHD) patients
Educate patients and caregivers in-clinic
Enable patients to perform dialysis independently at home
Train patients and caregivers on HHD procedures and self-care
Manage a caseload of HHD patients
including assessments and care planning
Monitor treatment progress
Provide on-call support
