Full Job Description
BAYADA Home Health Care is seeking a Registered Nurse, RN for a full time position with our new Central NJ Senior Living team - a Medicare Certified home health office. This office provides best-in-class care to adult and geriatric clients living in communities that include assisted living, independent living, senior congregate living, 55+ communities, and intermediate care facilities for clients with intellectual disabilities throughout Somerset, Middlesex and Monmouth Counties. *This position may also include seeing clients in the traditional home health community setting. Registered Nurse (RN) Responsibilities: Follow a designated care plan in accordance with patient's needs Make home visits to clients in designated geographic territories Perform assigned duties, including administration of medication, wound care, treatments, and procedures Monitor clients' conditions; reporting changes to Clinical or Client Services Manager Follow up with, execute, and properly document doctors' orders Perform client assessments as necessary Case management and coordination Accurately document observations, interventions, and evaluations pertaining to client care management and services provided, utilizing a state-of-the-art touch pad tablet Qualifications for a Registered Nurse (RN): A current license as a Registered Nurse
