Must Have
Current Texas Registered Nurse (RN) license, Three years of clinical nursing experience, Current Texas driver's license, CPR Required

Previous home health experience, Homecare Homebase experience
What You'll Do.
Perform comprehensive assessments in patient's home
Complete skilled documentation through Homecare Homebase
Evaluate patient and home health environment
Perform skilled home health clinical documentation
Communicate visit care plan with physician
Communicate visit care plan with care team members
Communicate visit care plan with patient and family
Serve as patient's advocate
Maintain ongoing communication with physician
Maintain ongoing communication with referral sources
Maintain ongoing communication with caregivers
Maintain ongoing communication with facility management
Facilitate coordination of care
Administer medications as prescribed by physician
Administer treatments as prescribed by physician
Provide direct patient care
Provide infusion therapy
Provide disease management
Provide direct supervision of nursing clinical care team
Participate in regular case conference
Participate in in-services
