What They're Looking For.
Must Have
Registered Dietitian as per Commission on Dietetic Registration, 1-year professional work experience in clinical nutrition, Current state licensure if applicable
Nice to Have
Board Certified Specialist in Renal Nutrition, Previous renal experience
What You'll Do.
Assess patient's knowledge of diet
Provide education appropriate to patient's learning needs
Provide patient specific detailed education
Complete nutritional assessment
Participate in interdisciplinary comprehensive assessments
Participate in development of individualized plan of care
Calculate diet prescription
Complete progress notes
Maintain complete and accurate documentation
Provide ongoing nutrition assessment
Provide outcome-oriented nutrition counseling
Identify malnourished patients
Identify patients at risk for malnutrition
Work collaboratively with interdisciplinary team
Review patient’s albumin level
Review patient’s body weight
Implement plan of care
Monitor adherence to nutrition therapy
Adjust therapy as needed
Document why patient was unable to achieve goals
Implement plan of care to address issues
Collaborate on Urea Kinetic Modeling recommendations
Collaborate on management of mineral metabolism
Collaborate on management of renal bone disease
Collaborate on management of anemia
Collaborate on management of iron status
Collaborate on management of nutritional status
Collaborate on management of psychosocial status
Collaborate on management of vascular access
Collaborate on management of modality
Collaborate on management of rehabilitation status
Collaborate on management of sodium/volume control
Adhere to FMCNA algorithm
Adhere to medical record policies
Report on nutrition QAI results
Participate in interdisciplinary QAI program
Collaborate and coordinate Pre-ESRD education
Coordinate plan of care for patient education
Identify candidates to refer for nutritional supplements
Review nutrition related lab results
Counsel patient and/or caregiver
Formulate appropriate action
Recommend treatment changes
Communicate with physician
Communicate with facility staff
Communicate lab results to providers
Communicate recommended treatment changes to providers
Participate in pertinent staff meetings
Consult with FMS Corporate Dietitian
Maintain and improve knowledge and skills
Maintain dietetic registration
Maintain continuing education hours
Collaborate with interdisciplinary team
Ensure work areas are safe and clean
Perform other nutrition management duties
Perform other related duties
