Where You'll Be This position will be remote with 10%-15% travel to provider offices. We are looking for candidates in the following areas: Texas Florida What You’ll Do The Recovery Patient Engagement Associate II will work collaboratively on the Patient Engagement Team and have shared accountability for enrolling and re-engaging children and caregivers into Imagine Pediatrics’ program. This role will be highly specialized and require strong critical thinking and creative problem-solving skills along with an aptitude for collaboration and teamwork. You will: Perform telephonic outreach to caregivers of attributed patients to enroll children in care, support seamless onboarding, and re-engage those who missed their initial onboarding visit or have not utilized services post-onboarding. Handle inbound phone calls from caregivers, health plan service coordinators, and hospital discharge planners as part of the enrollment and re-engagement process.
