What They're Looking For.
Must Have
High school diploma or GED equivalent, Certified Medical Assistant, Certified Pharmacy Technician, or equivalent experience in a healthcare setting, Intermediate to advanced level keyboarding skills (at least 35 WPM, touch typing, formatting documents)
Nice to Have
1+ years of experience in a healthcare setting, Prior experience using an Electronic Health Record (EHR), Previous call center or telephonic experience, Experience with Medicare Advantage plans, insurance, and medication adherence, Bilingual English/Spanish
What You'll Do.
Perform outbound patient calls regarding clinical programs
Address medication adherence and closing gaps in care
Interpret patient medical history
Document patient care activities
Perform medication reconciliation
Request medication refills per clinic policy
Assist patients with medication access barriers
Identify patients needing referrals
Follow up with patients
Connect with patients before and after primary care visits
Schedule appointments and testing
Maintain knowledge of quality measure requirements
Assist other quality departments
How You'll Work.
Team & Collaboration
Partner closely with quality, population health, and pharmacy teams; Serve as liaison between patient, clinic, and pharmacy; Actively participate as a member of the health care team; Coordinate with pharmacy staff, primary care office staff and/or providers
