Must Have

Bachelor's degree in Social Work, Bachelor's degree in Social Services, Bachelor's degree in Human Services, Bachelor's degree in Special Education, Minimum 2 years experience with IDD, Valid driver's license, Valid auto insurance, Reliable transportation, At least 18 years of age

Nice to Have

Master's degree a plus, 1-year experience with Master's degree, Supervisory/management experience preferred, Experience writing person-centered plans preferred

What You'll Do.

Coordinate IDT meetings

Assist people to identify needs

Communicate needs for services

Supervise person-centered plans

Integrate person-centered plans

Coordinate person-centered plans

Monitor progress towards outcomes

Initiate periodic reviews

Initiate investigations

Initiate modifications

Solicit feedback from people

Supervise person-centered plan

Write person-centered plan

Oversee residential activity

Ensure people are healthy

Ensure people are safe

Visit vocational centers

Visit residential homes

Assess plan implementation

Report to Administrator

Maintain communication with colleagues

Provide feedback to colleagues

Standardize procedures

Expedite plan implementation

Improve employee performance

Ensure quality assurance improvements

Ensure survey improvements

Serve as link to families

Serve as link to guardians

Gain approval for services

Perform other duties as assigned
