What You'll Do.
Deliver direct physical therapy services
Follow individualized plans of care
Assist patients with therapeutic exercises
Assist patients with gait training
Assist patients with balance
Assist patients with functional mobility
Support patients with transfers
Support patients with strengthening
Support patients with activities of daily living
Utilize therapeutic modalities
Educate patients and caregivers
Monitor patient progress
Report patient progress
Maintain accurate documentation
Communicate with Physical Therapist
Communicate with care team
Participate in case conferences
Participate in care coordination
Work closely with nurses
Work closely with therapists
Participate in team meetings
Participate in in-services
Participate in ongoing training
Uphold clinical standards
Contribute to quality improvement
