What They're Looking For.
Must Have
Active, unrestricted Texas PT license, Minimum 2 years of home health experience, Clean disciplinary record, No licensure encumbrances within the last 5 years, Current CPRLS certification, Current Texas driver's license, clinical assessment skills, OASIS knowledge, Patient, encouraging communication style with the ability to give constructive feedback
Nice to Have
Prior experience as a preceptor, clinical mentor, or educator in any setting, COS-C certification, Familiarity with Homecare Homebase (HCHB), Familiarity with PointCare, OASIS-E2 trained, HCS-D certification, wound care certification, Experience training clinicians who are new to home health
What You'll Do.
Precept and coach newly hired clinicians
Provide hands-on mentorship and support
Evaluate and sign off on clinical competency
Assist with skills lab training sessions
Administer and support end-of-orientation assessments
Serve as a resource for new joiners
Maintain a reduced patient caseload
Maintain quality standards
Model best practices in OASIS documentation
Identify and escalate concerns
Participate in AI charting product improvement focus groups
