What They're Looking For.
Must Have
High school diploma or equivalent, Previous experience working with emotionally and behaviorally challenged adults, Experience working in a recovery-oriented program, Experienced and proficient in using an electronic health record system, Ability to travel in the local area, Perform field work, Validated driver’s license, Dependable vehicle to travel between sites, Provide offsite services, Actively working toward certification provided by the Commission on Rehabilitation Counseling
Nice to Have
Associates Degree or higher
What You'll Do.
Serve as liaison between PRP and MH Departments
Coordinate and process referrals
Perform monthly chart reviews
Audit record keeping in SAMMS
Conduct initial assessment
Develop individualized treatment plan
Implement delivery of services
Teach and instruct clients
Promote individual wellness
Assist in restoration of self-care skills
Facilitate culturally relevant activities
Provide services through face to face interactions
Provide services through telehealth
Perform documentation requirements
Facilitate monthly collaborations with therapist
Maintain clinical records
Compile documentation required by CCG
Participate in staff meetings
Participate in program clinical team meetings
Participate in clinical supervision sessions
Participate in in-service training
Ensure confidentiality regarding client issues
Maintain case files within EHR

Team & Collaboration
Assuring consistent collaboration and flow of referrals between PRP and MH Departments; Coordinate referral process with Revenue Cycle Staff; Assure consistent communication between departments; Facilitate monthly collaborations with client’s assigned mental health therapist; Facilitate monthly collaborations with client’s treatment team
