What You'll Do.

Support program coordination across 15 RHTP initiatives

Plan and schedule regular virtual and in person meetings

Serve as the day-to-day primary point of contact for subrecipients

Compile consolidated quarterly and annual program data

Provide quality assurance (QA) and validation checks on data

Monitor 15 subgrantees’ performance and budget adherence

Conduct periodic audits to ensure alignment with project goals

Assist monitoring and evaluation team to ensure program outputs are clear

In the United States, ThinkWell applies this same systems-strengthening approach to domestic public health challenges—partnering with state and federal agencies to design, implement, and evaluate transformative programs. About the Role ThinkWell seeks a highly organized, entrepreneurial, and impact-oriented Project Manager to support our engagement on the Delaware Rural Health Transformation Program (RHTP)—a landmark federal initiative funded by the Centers for Medicare & Medicaid Services (CMS) supporting up to $1 billion in rural health investment over five years. The RHTP aims to transform health care delivery across rural Sussex and Kent Counties, Delaware, through 15 coordinated initiatives spanning crisis stabilization, behavioral health integration, primary care redesign, workforce development, and community-based prevention. The Project Manager will serve as ThinkWell’s primary managerial lead for this engagement and the main point of contact with the state of Delaware’s 15 RHTP subrecipients. This role sits at the intersection of grant compliance, multi-stakeholder coordination, and monitoring and evaluation, with direct exposure to a diverse ecosystem of rural health system partners.
