Must Have
Graduate of an accredited school of nursing, Current licensure in State, CPR certification, Licensed driver with automobile
Nice to Have
1-2 years acute care experience, 1-2 years home care experience, Bachelor's degree, 1 year hospice care experience
What You'll Do.
Complete initial patient assessment
Complete ongoing patient assessment
Complete family assessment
Provide physical assessment
Provide history of illness
Utilize nursing process
Evaluate patient status
Evaluate family needs
Initiate plan of care
Determine nursing diagnosis
Incorporate palliative actions
Include patient in planning
Include family in planning
Initiate preventive procedures
Initiate rehabilitative procedures
Administer medications
Administer treatments
Provide health care instructions
Assist patient with ADLs
Facilitate patient self-sufficiency
Facilitate comfort care
Coordinate patient care
Complete clinical notes
Maintain clinical notes
Submit clinical notes
Record pain management changes
