Plan and develop effective patient and family training.

Complete and submit required documentation.

Provide skilled interventions as outlined by AOTA.

Treat a diverse population.

Participate in patient team conferences.

Facilitate patient goals through Goal Attainment Scaling.

Communicate with client and family to develop rapport

and transition to next level of care.

Ensure appropriate recommendations are made for DME needs for safe transition.

Document information clearly and completely utilizing correct forms/systems and in a timely manner.

Carry out interventions that are supported by evidence-based practice standards.
