Graduate of an accredited school of Advanced Practice Registered Nursing, Current Advanced Practice Registered Nurse License in the state of employment, End-of-Life Nursing Education Program (ELNEC) – all 8 modules completed within 18 months of hire, Valid driver’s license, good driving record, reliable transportation, Ability to lift up to 50 pounds, Must clear criminal background check, physical, drug screening

What You’ll Be Doing Perform comprehensive nursing assessments for hospice patients Assess signs and symptoms of discomfort, including pain, and take prompt, appropriate action Initiate and manage individualized plans of care for patients and families Provide direct face-to-face visits for referred and existing patients Communicate with physicians regarding patient updates and care plans Maintain knowledge of state and federal regulations and JCAHO standards (if applicable) Participate in Quality Assessment and Performance Improvement (QAPI) programs Ensure all services comply with hospice standards and regulatory requirements Qualifications Graduate of an accredited school of Advanced Practice Registered Nursing MSN or Doctoral Degree required Current Advanced Practice Registered Nurse License in the state of employme
