Valid driver's license, Valid Auto Insurance

What They're Looking For.

Must Have

Minimum of two years social work experience in a healthcare setting or social services agency, Current Social Worker license (required), Valid driver's license (required), Valid Auto Insurance (required), Bachelor's degree in Social Work with a minimum of 1 year of healthcare experience

Nice to Have

Master’s degree in Social Work preferred, Hospice experience a plus

What You'll Do.

Provide psychosocial services to terminally ill patients and

Make contact with patient

Complete psychosocial assessment within 24 hours of completion

Assess patient and family psychosocial needs

Assess pre-bereavement/anticipatory grief needs

Implement Plan of Care provisions to meet needs

Re-evaluate patient's and family's psychosocial needs

Participate in weekly interdisciplinary team meetings

Assist with making funeral arrangements if requested

Provide counseling during terminal illness and at time
