What You'll Do.
Assess counseling needs
Determine level of care
Review clinical records
Develop treatment plans
Deliver individual counseling
Deliver group counseling
Conduct periodic reviews
Revise treatment plans
Develop discharge plans
Develop transfer plans
Coordinate with referral sources
Participate in treatment team
Provide clinical consultation
Provide care coordination
Document counseling sessions
Document follow-up services
Attend staffing meetings
Respond to psychiatric crises
Adhere to agency policies
Adhere to reporting requirements
Maintain ethical approach
Address addiction issues
Address trauma issues
Address dual diagnosis issues
