What You'll Achieve.
Ensure child safety; Facilitate transition to an educational setting; Assist children to successfully access preschool.; Ensure that all resources are used appropriately to meet the client's developmental needs.

Mitigate complex support needs; Develop, implement, and monitor a support plan that addresses the unique needs, choices, strengths, and risks of the child to successfully access preschool.; Identify gaps in available community services and make data-driven recommendations for cost-effective solutions.
Must have a valid driver license, Must pass a criminal background check, All employees must reside in Oregon or the county cannot support ongoing telework from other locations.

Must Have
Knowledge of the public service system for developmental disabilities services in Oregon, A bachelor’s degree in behavioral science, social science, or a closely related field OR A bachelor’s degree in any field and one year of Human services related experience OR An associate’s degree in a behavioral science, social science, or a closely related field and two (2) years of human services related experience OR Three (3) years of human services related experience, Diversity and Equity: Ability to promote a culture of respect, inclusiveness, and appreciation of diverse perspectives, backgrounds, and values, Must have a valid driver license, Must pass a criminal background check

What You'll Do.
Conduct comprehensive
advanced assessments to determine the complex and often multi-system support needs of children identified by the preschool provider and preschool for all inclusion coordinator.
and monitor a support plan that addresses the unique needs
and risks of the child to successfully access preschool.
Monitor and document the use of behavior services to ensure compliance with Department approved behavior model
Positive Behavior Support (PBS)
Oregon Intervention System (OIS) and OARs regarding psychotropic medications.
Develop and implement protocols with partner agencies for the identification of appropriate high-risk individuals
development of collaborative care plans
and effective service delivery.
Identify gaps in available community services and make data-driven recommendations for cost-effective solutions.
Assess and facilitate services to assist assigned children in transitioning to an educational setting.
Provide technical assistance to service coordinators (case manager 2 classification) and their supervisors to approve appropriate resources to access preschool.
Monitor all services and supports via visits to preschool providers sites
high frequency of contact with assigned children’s parents and other community partners to assist children to successfully access preschool.
Ensure that all resources are used appropriately to meet the client's developmental needs.
Maintain accurate case records and document all actions taken in accordance with federal
