What You'll Achieve.
Ensure practices meet goals set forth in mutually developed Practice Transformation Plans; Support and enhance financial performance across performance metrics; Support and enhance quality performance across performance metrics
Industry & Context.
Must be based in Oregon, Sitting for prolonged periods of time, Extensive use of computers and keyboard, Occasional walking and lifting may be required, Ability to travel to practices within southern Oregon approximately 25% across the year
What They're Looking For.
Must Have
Resident of Oregon, 4-6 years of significant and relevant work experience in medical practice management in lieu of educational requirements may be accepted, particularly with significant administrative experience in a clinic setting, 3 years of experience with Electronic Health Records (EHR) for clinical/practice management processes
Nice to Have
Excellent oral and written communication skills (i. e. , public presentation skills), organizational and project management skills, team-building skills, Familiarity with process mapping and work-flow analysis tools, Excellent computer skills and willingness to learn additional software applications

Execute ACO initiatives
Implement and support wellness visits
Implement and support transitions of care
Implement and support active management
Ensure practices meet goals
Assist in training office staff
Assist in training physicians
Assist in training providers
Provide continual assessment of practice’s training needs
Provide continual assessment of staff’s training needs
Develop lasting relationships with physicians
Develop lasting relationships with providers
Develop lasting relationships with healthcare system executives
Nurture lasting relationships with physicians
Nurture lasting relationships with providers
Nurture lasting relationships with healthcare system executives
Listen to practices concerns
Relay important information to Manager/Leads
