Must Have
Minimum 3 years of experience in primary care operations, practice management, healthcare operations, or a directly related role, Clinical background — RN or equivalent clinical experience in a primary care setting are required, Experience working within an ACO, MSSP, or value-based care model — delegated services, managed care, or population health environm
What You'll Do.
Serve as primary point of contact
Build and maintain relationships
Conduct proactive outreach
Maintain consistent communication cadence
Schedule performance touchpoints
Act as advocate for practices
Communicate operational needs and risks
Facilitate ongoing engagement
Ensure consistent high-quality engagement
Deliver monthly engagement scorecards
Lead performance review conversations
Schedule quarterly performance meetings
Translate scorecard data into actionable steps
Monitor operational performance
Monitor engagement metrics
Monitor utilization of Guidehealth services
Identify practices at risk
Escalate underperformance with context
Support practices in developing improvement plans
Facilitate weekly pillar huddles
Review current practice state
Coordinate cross-functional responses
Serve as liaison between practices and clinical pillars
Coordinate AWV workflow implementation
Coordinate CM embedded rollout
Coordinate PAC workgroup activities
Collaborate with Clinical Operations
Collaborate with Technology teams
Collaborate with Quality teams
Collaborate with Product teams
Communicate practice-level barriers and needs
Track resolution to completion
Maintain accurate practice data in HubSpot CRM
Update contact records
Update engagement status
Update network tracking items
Document meeting notes
Distribute action item summaries
Develop standard operating procedures
Maintain workflow documentation
Identify opportunities to standardize
Improve implementation processes
