What They're Looking For.

Must Have

Minimum 3 years of experience in primary care operations, Minimum 3 years of experience in practice management, Minimum 3 years of experience in healthcare operations, Clinical background — RN or equivalent clinical experience, Experience working within an ACO, Experience working within an MSSP, Experience working within a value-based care model

Nice to Have

Delegated services experience, Managed care experience, Population health experience

What You'll Do.

Serve as primary point of contact

Build and maintain relationships

Conduct proactive outreach

Maintain consistent communication cadence

Schedule performance touchpoints

Act as advocate for practices

Communicate operational needs

Address feedback and risks

Facilitate ongoing engagement

Support long-term retention

Ensure high-quality engagement

Deliver monthly engagement scorecards

Lead performance review conversations

Schedule quarterly performance meetings

Translate scorecard data

Develop actionable next steps

Monitor operational performance

Monitor engagement metrics

Monitor utilization of services

Identify practices at risk

Escalate underperformance

Support practices in developing plans

Track improvement plans

Facilitate weekly pillar huddles

Review current practice state

Coordinate cross-functional responses

Serve as liaison between practices

Coordinate AWV workflow implementation

Coordinate CM embedded rollout

Coordinate PAC workgroup activities

Collaborate with Clinical Operations

Collaborate with Technology teams

Collaborate with Quality teams

Collaborate with Product teams

Ensure seamless service delivery

Communicate practice-level barriers

Communicate practice-level needs

Track resolution to completion

Maintain accurate practice data

Update contact records

Update engagement status
