**About the position** The population health nurse leverages data analytics to support chronic disease management initiatives and collaborate with interdisciplinary teams to develop and implement evidence-based interventions. The ideal candidate is a proactive, data-informed clinician with strong communication skills and a passion for improving systems of care. This role requires the ability to think strategically while delivering patient-centered interventions that drive measurable improvements in health outcomes and patient experience. **About Outpatient Clinics** Corewell Health operates over 300 outpatient clinics across Michigan, bringing a multitude of care options to the communities that we serve. There are over 40 different types of primary care and specialty practices, which allows those who work in these clinics the opportunity to learn and explore various interests or to specialize in one area. **Scope of work** Serves as a population health subject mater expert in the practice. Plans and prioritizes care for individuals and population of patients, focusing on strategies that will promote optimal health within populations and directs patients to the right care team member at the right time. Demonstrates expertise, current knowledge in nursing care and management of a sub population of patients of varying complexity and seeks to improve patient, family and health systems / community outcomes through the application of educational concepts / skills and preventive care in a managed care environment. **Qualifications** * Bachelor's Degree or equivalent in nursing or a healthcare related field Required * 3 years of relevant experience clinical nursing and or case management/managed care or related field Required * LIC-Registered Nurse (RN) – State of Michigan Upon Hire Required * CRT-Basic Life Support (BLS) - AHA American Heart Association Ambulatory 90 Days Required or CRT-Basic Life Support (BLS) - ARC American Red Cross Ambulatory 90 Days Required ## How C
