Full Job Description

# **Become a part of our caring community** Humana's Primary Care Organization is a provider of senior-focused, value-based care, with more than 340 centers across 15 states under the CenterWell and Conviva brands. This is a full-time, on-site, patient-facing role. You report to the Market Chief Medical Officer (CMO), or Associate Medical Director (AMD), and are responsible for both direct patient care and clinical leadership for the Anderson, SC clinic. ## About the Role You will provide primary care for senior patients in a value-based care model while leading a team of clinicians to lead quality, outcomes, and patient experience. You will balance patient care with leadership responsibilities, supporting clinicians, improving performance, and advancing a patient-centered model of care. ## What You'll Do ### Clinical Care * You will provide primary care for senior patients, including those with complex and chronic conditions * You will see patients and manage ongoing care, typically spending 50–80% of your time in direct patient care * You will complete accurate documentation to support care and outcomes * You will manage a clinical inbox, including labs, messages, and follow-ups * You will coordinate care with specialists and interdisciplinary care teams * You will apply Medicare risk adjustment principles through accurate condition documentation ### Clinical Leadership * You will lead a team of Physicians, Nurse Practitioners, and Physician Assistants * You will coach clinicians using data, performance dashboards, and patient outcomes * You will participate in hiring, onboarding, and retention of clinical staff * You will provide performance feedback, complete evaluations, and support professional growth * You will foster a culture of collaboration, accountability, and continuous improvement ### Quality & Performance * You will improve performance in patient experience, quality, and clinical outcomes * You will monitor and improve important metrics such as: * Net
