What They're Looking For.
Must Have
Active, unrestricted Texas PT license, Minimum 2 years of home health experience, Clean disciplinary record, No licensure encumbrances within the last 5 years, Current CPRLS certification, Current Texas driver's license, clinical assessment skills, OASIS knowledge
Nice to Have
Prior experience as a preceptor, clinical mentor, or educator, COS-C certification, Familiarity with Homecare Homebase (HCHB), Familiarity with PointCare, OASIS-E2 trained, HCS-D certification, wound care certification, Experience training clinicians who are new to home health
What You'll Do.
Precept and coach newly hired clinicians
Provide hands-on mentorship and support
Evaluate and sign off on clinical competency
Assist with skills lab training sessions
Administer and support end-of-orientation assessments
Serve as a resource for new joiners
Maintain a reduced patient caseload
Maintain quality standards
Model best practices in OASIS documentation
Identify and escalate concerns
Participate in AI charting product improvement focus groups

- Role: Field Supervisor (PT) - Comp: Salaried, exempt - PT: $109,200 annualized at 25 points/week expectation - Mileage and travel reimbursement in addition to base - Schedule: Full-time Monday–Friday - Location: Field-based (Texas) - Reports to: Clinical Manager / Branch Director / Director of Clinical Education Adaptive Home Health is building a higher-acuity, patient-centered, skilled home health model across Texas. Our ultimate mission is to dramatically improve patient access to home health care. The Field Supervisor role is how we make that mission scale: you guide newly hired and transitioning clinicians through real-world home health practice, validate their clinical competency, and model the standard of care every patient deserves. WHO WE ARE: We build technology to better support our field clinicians — faster and more supported. Our clinicians are the customers of our engineering team. Our charting product is designed to reduce documentation burden and keep visits moving, and our scheduling product (launching soon) will make it easier to coordinate visits, reduce last-minute changes, and protect clinician time. For Field Supervisors specifically, you'll have direct input into how those tools evolve through AI focus groups — your frontline experience shapes the product roadmap. If you have strong clinical judgment, love developing other clinicians, and want autonomy with real organizational impact, this role gives you a meaningful blend of patient care, teaching, and product influence.
