- Role: Physical Therapy Assistant (PTA) - Schedule: Full-time Monday–Friday + occasional weekends (as needed) - Location: Field Based - Compensation (Productivity Points): $58.00 / point | $75,000-$100,000/year WE'RE FIXING HOME HEALTHCARE, AND WE'RE BUILDING OUR PRESENCE IN DETROIT Adaptive is building an AI-native home healthcare platform that enables clinicians to focus on delivering best-in-class care for their patients by leveraging software and innovative support solutions. We’re building our team in Detroit with plans to quickly develop an exceptional reputation of delivering excellent patient care. THE MISSION OF THIS ROLE Deliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on. OBJECTIVES 1. Carry a full, high-quality caseload: ramp to ~30 productivity points/week at target visit quality. 2. Own your patients' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality. 3. Be a referral-source ambassador: every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent. 4. Set the standard others copy: your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows. WHAT YOU'LL OWN - Deliver skilled PTA visits in the home under the supervising PT’s plan of care. - Execute treatment plans with a focus on functional mobility and measurable progress: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention. - Set/track objective progress within PTA scope and communicate updates/concerns to the supervising PT. - Drive multidisciplinary coordination: escalate changes in condition, align on orders, and keep the care team moving. - Coach patients and caregivers with practical home programs that a
