Issues and provides instruction in the use of adaptive equipment
Confers with therapist directly
and/or interdisciplinary team members regarding patient response to therapy and any concerns
Provides PT gait and ambulation training
Instructs in the use of assistive devices
Provides instruction in safe transfer using appropriate body mechanics and equipment
Participates in discharge planning and provides timely and adequate patient and family teaching to facilitate the discharge plan
Coordinates patient schedule with team members
readily and effectively adjusts schedule to meet changes in census and patient needs

**SUMMARY** Under the direction of a NYS licensed Physical Therapist, the Physical Therapy Assistant (PTA) provides and documents physician-ordered occupational therapy services. The PTA provides superior customer service by modeling the Unity Brand Promise and Core Values. **STATUS:** Full Time **LOCATION:** Unity Hospital - 1555 Long Pond Rd, Rochester, NY 14626 **DEPARTMENT:** Physical Therapy **SCHEDULE:** Day shift, 8-hr. shifts, 8 am - 4:30 pm; weekend rotation requirement 1-2 weekend shifts per month **RESPONSIBILITIES** * Within standards of practice established for a PTA, completes and documents (within agency standards) related to patient care, including visit notes, renewal orders, and verbal orders. Includes progress or lack of goal achievement in all charting. * Implements treatment programs individualized to meet patient needs and discharge plans: issues and provides instruction in the use of adaptive equipment. Confers with therapist directly, physician, and/or interdisciplinary team members regarding patient response to therapy and any concerns. Provides PT gait and ambulation training.
