- Role: Physical Therapist (PT) - PRN - Schedule: Part-time flexibility (pick up visits based on your availability; weekdays and/or weekends as needed) - Location: Field Based - Compensation (Productivity Points): $75.00 / point WE'RE FIXING HOME HEALTHCARE, AND WE'RE BUILDING OUR PRESENCE IN DETROIT Adaptive is building an AI-native home healthcare platform that enables clinicians to focus on delivering best-in-class care for their patients by leveraging software and innovative support solutions. We’re building our team in Detroit with plans to quickly develop an exceptional reputation of delivering excellent patient care. THE MISSION OF THIS ROLE Deliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on. OBJECTIVES 1. Carry a high-quality caseload 2. Own your patients' outcomes: drive low avoidable-hospitalization rates and strong patient-experience scores; participate in case conferences/team huddles to improve our quality. 3. Be a referral-source ambassador: every discharge planner, ALF, and physician you touch should want to send the next patient to us because you were responsive and excellent. 4. Set the standard others copy: your charting, cadence, and judgment become the template we onboard the next cohort against as the pod grows. WHAT YOU'LL OWN - Own the caseload as a case manager: you’re accountable for the patient’s therapy plan, progression, and outcomes — not just individual visits. - Complete comprehensive in-home assessments and deliver skilled PT visits. - Build and execute a clear plan of care: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention. - Set measurable goals, track objective progress, and adjust treatment quickly when patients stall. - Drive multidisciplinary coordination: communicate changes in condition, align on orders, and keep the care team moving. - Manage treatment and out
