What They're Looking For.
Must Have
Active Texas PT license (in good standing), clinical judgment and comfort working independently in the field, Comfortable escalating changes in condition to RN/MD and documenting rationale clearly, communication and documentation, Reliable follow-through on orders, frequencies, and patient goals, Current driver’s license and comfort with daily travel across assigned territory, Able to carry/transport basic therapy equipment as needed, Competency in core home health PT scope, Safety-first mobility work: transfers, gait training, balance/fall prevention, assistive device recommendations, Strengthening, functional mobility, and home safety assessment/modification education, Patient/caregiver coaching with practical home programs
Nice to Have
Prior home health experience, Homecare Homebase (HCHB) experience (or similar home health EMR), Deeper clinical experience in high-signal PT populations, Experience supervising/mentoring PTAs and collaborating on care plans, organization and time management
