What They're Looking For.
Must Have
Current Physical Therapy license within the State of practice, BLS certification required, Ability to become proficient with computer software applications commonly used in an automated patient care environment, Arizona Fingerprint Clearance Card at the time of hire

What You'll Do.
Evaluate patient's functional abilities and limitations
Determine if intervention is needed
Establish a plan of care and treatment goals
Complete comprehensive evaluation within time frame
Identify and prioritize key limiting factors
Establish treatment goals that are functional
Collaborate with patient and family/caregiver
Initiate discharge planning
Recommend additions to or modifications of referring orders
Implement individualized treatment plan
Provide skilled treatment in a safe manner
Assess ongoing effectiveness of treatment
Modify plan of care as indicated
Involve patient and family/caregiver in treatment
Provide ongoing education to physician
Document according to professional practice guidelines
Complete documentation within time frames
Document evidence based and individualized treatment techniques
Reassess treatment goals according to department policies
Communicate pertinent patient information
Communicate evaluation/treatment results and patient progress
Communicate in a clear and timely manner
Contact physicians for routine or difficult problem solving
Defend treatment rationale with standard practices
Exhibit adaptability in assignments
Take initiative to assist in other patient care areas
