What They're Looking For.
Must Have
Current NJ Physical Therapist license, Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association
Nice to Have
Dementia Care Certified, Experience working with the IDD population - of any age, Experience working with clients with Alzheimer’s / Dementia, Experience with assessing hoyer lifts and mechanical lifts, Experience with assessing durable medical equipment, Experience writing a letter of medical necessity, Solid computer prior experience with electronic medical records (EMR)
What You'll Do.
Make home visits to clients in designated geographic territories
Perform diagnostic tests and measurements
Implement integumentary protective interventions
Develop and implement appropriate individualized care plans
Continually assess and revise the Physical Therapy - PT care plan
Participate with nursing in the multidisciplinary care plan
Educate and instruct clients
or other client representatives
Accurately document observations
and evaluations pertaining to client care management and services provided

Integral member of a multi-disciplinary health care team; Participate with nursing in the multidisciplinary care plan
