Whether it’s a neurological issue or a physical trauma, you encourage, comfort, and ready your patients for therapy, interacting with physicians, case managers, nurses, and families as part of a team care approach. And we’re excited to get to know you and find out what you will bring to this critical rehabilitation role. **Your Everyday** * Take the lead: prioritizing, coordinating, and delegating all physical rehabilitation care touchpoints for your patients. * Interact with other members of the rehabilitation team to coordinate goals and improve quality of care. * Ensure patient physical therapy care and activities consistent with the medical plan of care. * Maintain and advocate a safe environment for everyone. Monitor and engage with your patients, families, and appropriate care team members for teaching and education opportunities, patient condition changes, or patient questions and concerns. * Ensure continuity of care, efficient throughput, consistent communications, and an exceptional experience for every patient. * Contribute to the team by assisting others without being asked.
