Must Have
Graduate of an ACPE accredited Doctor of Pharmacy degree OR Bachelor of Science in OR Foreign Pharmacy Graduate Examination Certification (FPGEC), Pharmacist Intern certification required prior to start of program, Pharmacist certification required before October 1 of the year starting the residency

Full Job Description
Your job is more than a job. The Pharmacy Resident PGY1 is responsible for working with multidisciplinary health care teams to achieve optimal outcomes. Compounds and dispenses medications and pharmaceutical supplies upon receiving physician orders. Reviews and evaluates drug regimens, which include the medication profile to identify issues that require pharmacy intervention or monitoring. Provides pharmacy services and drug information to customers as necessary. **_GENERAL DUTIES_** * Functions as a pharmacist to perform basic responsibilities in compliance with Board of Pharmacy, Drug Enforcement Agency (DEA), and The Joint Commission (TJC) * Prepares, dispenses, and manages medications to support safe and effective drug therapy for patients. * Participates in patient care rounds with multi-disciplinary teams to make pharmaceutical interventions and evaluate patient progress. * Conducts and evaluates medication histories, assesses compliance and suggests modifications so as to achieve desired outcomes. * Collaborates with the healthcare team to provide safe and effective patient care to a diverse range of patients, including those with multiple co-morbidities, high-risk medication regimens and multiple medications following a consistent patient care process. * Provides general and patient specific medication information to patients, providers, nurses and other health care professionals. Interacts routinely with physicians and nurses to provide optimal pharmaceutical care to patients. * Counsels patients in medication attributes and the proper use of medications * Participates in collection of data for medication utilization evaluations (MUEs) * Demonstrates the ability to manage formulary and medication-use processes, as applicable to the organization * Serve on hospital committees and present material to committee members as needed (e.g. Pharmacy & Therapeutics Committee, Antibiotic Stewardship Committee, etc.) * Complete a research project by demonstrating the a
