What They're Looking For.
Must Have
Bachelors of Science Degree in Pharmacy or Doctor of Pharmacy degree, Certificate of competency from the Pharmacy Examination Board of Canada, Active registration with the Ontario College of Pharmacists (Part A), Certificate of competency from an accredited smoking cessation program, Certificate of competency from an accredited flu vaccination injection program
Nice to Have
Hospital Outpatient Pharmacy practice experience, Hospital pharmacy residency, Experiences in handling blister pack
What You'll Do.
check and dispense all prescribed medication orders
resolve and prevent drug related problems
Monitor drug therapy for effectiveness
toxicity and adherence
Conduct medication reconciliation
Follow up on medication incidents
Promote safe medication practices on the unit
Respond to drug information questions
Providing direct and/or functional supervision of technician staff
