Must Have
Licensure, or licensure-eligibility, to practice pharmacy in the state of Ohio, Doctor of Pharmacy
Nice to Have
Homecare, Home Infusion or Hospital experience or internship
What You'll Do.
Dispenses medications for NCHH
calculates appropriate dosing
Oversees pharmacy technicians and interns
Provides input on training
Provides input on performance evaluations
Coordinates and collaborates with patients
Develops and maintains a medication list
maintains an individualized pharmacy plan of care
Documents all clinical and operational patient care activities
Provides drug information
How You'll Work.
Team & Collaboration
collaborates with patients, their families, nursing providers, discharge planners and/or prescribers; Provides drug information to health professionals, patients, and caregivers
Communication Scope
communication and interpersonal skills required in providing education to patients/families and patient care team as well as in interactions with students, the public, pharmaceutical representatives, and representatives from external institutions
