An applicant that has worked in a sterile compounding environment and/or overseen sterile compounding activities, **is preferred.** **Job Summary:** Provides pharmaceutical care to patients, verifies medication orders, compounds and dispenses pharmaceuticals in accordance with federal, state and local regulations and hospital policy. Serves as a resource to other healthcare professionals including pharmacy students and residents and ensures that patient care meets standards of safety and effectiveness. Oversees the duties of the pharmacy technicians and interns. **Job Responsibilities and Requirements:** PRIMARY RESPONSIBILITIES * Ensures safety and appropriateness of medication therapy through order verification, dispensing, home medication reconciliation, chart review, patient counseling, lab value monitoring and dose/therapy adjustment as applicable. Documents medication related plans and interventions in electronic medical record. * Provides clinical consultation delivering best-practice, evidence-based and complete information to practitioners.
