**It 's more than a career, it's a calling** MO-SSM Health DePaul Hospital - St. Louis **Worker Type:** Regular **Job Highlights:** **Scope:** Oversees sterile compounding activities and regulatory compliance with some staffing in the central pharmacy (~80% sterile compounding/20% staffing on average) **Schedule: Variable.** Team member **will mostly be days** while overseeing sterile compounding, but while staffing on the weekends may be days or evenings Shifts: **Days (7:30am-4pm)** or evenings (2pm/3pm-1030pm/1130pm); will also be in the rotation to work open overnight shifts, _if necessary_ Applicants need inpatient hospital pharmacy experience. An applicant that has worked in a sterile compounding environment and/or overseen sterile compounding activities, **is preferred.** **Job Summary:** Provides pharmaceutical care to patients, verifies medication orders, compounds and dispenses pharmaceuticals in accordance with federal, state and local regulations and hospital policy. Serves as a resource to other healthcare professionals including pharmacy students and residents and ensures that patient care meets standards of safety and effectiveness. Oversees the duties of the pharmacy technicians and interns. **Job Responsibilities and Requirements:** PRIMARY RESPONSIBILITIES * Ensures safety and appropriateness of medication therapy through order verification, dispensing, home medication reconciliation, chart review, patient counseling, lab value monitoring and dose/therapy adjustment as applicable. Documents medication related plans and interventions in electronic medical record. * Provides clinical consultation delivering best-practice, evidence-based and complete information to practitioners. Makes recommendations and encourages safe and cost-effective pharmacotherapy for patients through multi-disciplinary collaboration. * Oversees the medication dispensing process to ensure accurate and timely delivery of optimal pharmacotherapy to patients.
