Must Have
B. Sc. Phm required, Licensed under Part A of the Ontario College of Pharmacists register required, Competency in medication order review, Competency in clinical assessment, Competency in therapeutic recommendations, Aseptic technique, Dispensing skills, Demonstrated high degree of accuracy, Demonstrated attention to detail, Excellent clinical problem recognition skills, Excellent problem-solving skills, Proficient in Microsoft Office programs, Able to manage multiple clinical responsibilities, Able to work in a fast-paced acute care environment, Well-organized, Self-directed, Able to set priorities, Able to manage time effectively, Able to interact patiently, Able to interact effectively, Able to interact professionally, Committed to clinical excellence, Committed to patient safety, Committed to evidence-based practice, Excellent verbal communication skills, Excellent written communication skills, Able to meet physical demands of the position, Satisfactory employment record, Satisfactory attendance record
Nice to Have
Hospital pharmacy residency preferred, Equivalent acute care clinical experience preferred, Demonstrated clinical pharmacy experience preferred, Proficiency with CPOE system preferred, Meditech Expanse proficiency preferred, EPIC proficiency preferred

Enter orders into Meditech Expanse
Perform checking responsibilities
Troubleshoot clinical issues
Troubleshoot drug distribution issues
Act as a resource for Pharmacy Technicians
Act as a resource for Students
Provide back-up coverage for staff
Perform medication order review on patient care units
Perform medication order review on departments
Investigate clinical problems
Investigate drug distribution problems
Perform medication reconciliation
Perform medication resolution
Liaise with other sites
