Whilst maintaining patient safety, the successful candidate will be responsible for the patient and prescription management for specialist homecare Pharmacy therapies. **Essential Duties & Responsibilities** * Be the Responsible Pharmacist to deliver prescription management and dispensing across all therapies in line with operational KPIs and standards, whilst maintaining patient safety. * Process the prescription receipt and conduct clinical screening/intervention in line with the standard operating procedures. * Accurately dispense prescriptions and ensure the timely reporting of near misses and dispensing errors. * Place product order and authorise prescription entries on the Patient Medication Record. * Formulate unlicensed medicines and raise the purchase order with the manufacturing provider in line with the patient & customer requirements within the contractual timeframes. * Communicate and collaborate with the relevant clinical teams to ensure patient adherence is being met in line with the prescription and escalate where appropriate. * Record and escalate adverse advents and inci
