What They're Looking For.
Must Have
Valid Personal Support Worker Certificate from an approved program, or RN/RPN Student with completed first year, or Internationally licensed Nurse, Midwife or Physician. Minimum 1 year experience with palliative patients. Must possess the physical ability to perform duties. G2/G License with access to a vehicle.
Nice to Have
Successful completion of Fundamentals of Hospice Palliative or Core Concepts of Fundamentals for, and additional relevant palliative education. Current CPR and First Aid Certificate. Current Police check suitable for working with the vulnerable sector.

How You'll Work.
Team & Collaboration
Works closely with the interdisciplinary palliative care team and health system partners to deliver integrated care. Communicates patient status and concerns to the Integrated Care Program Supervisor.
