We are currently seeking full and part time Personal Support Workers (PSW) to be part of a dedicated and specialized team that will deliver a highly integrated, one team, community palliative care model for the Mississauga community. Working in lockstep with health system partners, you will be part of a skilled and collaborative team that supports people and their families throughout their palliative care journey. You will support patients, families and caregivers –enhancing quality of life, comfort, dignity and security in patients’ end of life journey. All direct patient care activities will be performed under the direction of an Integrated Care Program Supervisor who is a nurse. * Working alongside the interdisciplinary palliative care team, you will provide specialized care as specified in the care plan, according to protocols, within scope of practice and in accordance with Spectrum Health Care’s policies and procedures. * Provide companionship and comfort, creating a calm and respectful environment tailored to the client's needs, preferences, and goals of care. * Assist person(s) with activities of daily living such as bathing, toileting, hygiene, skin care, light meal preparation, feeding and transfers. * Perform light housekeeping dut
