Role Scope: The Physical Therapist Assistant (PTA) provides high-quality rehabilitative support for participants enrolled in the Habitat Health PACE program (Program of All‑Inclusive Care for the Elderly). Working under the supervision of a licensed Physical Therapist, the PTA delivers therapeutic interventions that promote mobility, safety, independence, and overall quality of life. This per‑diem role offers flexible scheduling while supporting participant needs across clinic, home, and day‑center settings in Sacramento. Key Responsibilities Therapeutic Interventions — Implement individualized treatment plans including therapeutic exercise, gait training, balance activities, and functional mobility tasks under the supervision of the PT. Participant Assessment Support — Observe, document, and report participant progress, functional changes, and safety concerns to the supervising PT. Interdisciplinary Collaboration — Coordinate with nurses, social workers, occupational therapists, and other PACE team member
