Criminal Record Check may be required, must have successfully completed their probationary period, have a good employee record along with satisfactory attendance in accordance with UHN's attendance management program (for current UHN employees)
What They're Looking For.
Must Have
Completion of a high school diploma
Nice to Have
Completion of recognized Human Services or Social Services diploma, Two years work experience in Mental Health and/or Substance Use, Lived experience with Mental Health challenges and/or Substance Use, Peer Support Certificate or training
What You'll Do.
client-centered care within the ACT model
including visiting clients in the community
helping maintain a safe environment and connecting clients with community resources and services.
Advocate for clients to help them access the supports they need
and provide education to clients
and families about their rights and how to access and maintain services.
Helps connect clients with social and recreational opportunities that match their interests and needs
including accompanying them to appointments
and community events when appropriate.
Supports and encourages client participation in social and recreational activities by helping plan
and explore different opportunities
and by sharing information about available events and programs.
Draws upon lived experience to provide invaluable knowledge to staff
students and the community at large
including imparting knowledge of different/appropriate types of techniques
