Full Job Description

Who We Are Imagine Pediatrics is a tech enabled, pediatrician led medical group reimagining care for children with special health care needs. We deliver 24/7 virtual first and in home medical, behavioral, and social care, working alongside families, providers, and health plans to break down barriers to quality care. We do not replace existing care teams; we enhance them, providing an extra layer of support with compassion, creativity, and an unwavering commitment to children with medical complexity. The primary location for this role is remote, the expected schedule requirement is 4x10s 8:00 am - 6:00 pm (Tuesday-Friday). Active LA, TN, MO, or TX licensure is required. What You’ll Do As an APP with Imagine Pediatrics, you influence and ensure the best clinical care for your patients. Our goal is to increase the number of safe days at home by ensuring kids and families get what they need when they need it—reducing unnecessary medical visits and hospitalizations. You leverage an integrated technology platform and are complimented by an entire interdisciplinary team including registered nurses, navigators, dietitians, pharmacists, psychiatrists, social workers, and therapists. You will:  Manage a panel of complex pediatric patients – Oversee care plans, collaborate with PCPs and specialists, and ensure comprehensive care coordination. Work in a multidisciplinary team – Work with pediatricians, nurses, pharmacist, dietitian and social workers to provide wrap-around care in a virtual-first model. Empower families – Educate and support parents to navigate their child’s health journey with confidence, ensuring long term health success. Deliver high-quality virtual care – Conduct virtual enrollment visits, acute escalations, sick visits, and post-hospital follow-ups, and even in-home virtual assessments supported by our Mobile Integrated Care team.
