What You'll Do.
Manage schedules for all patients
Edit schedule for agents calling in sick
Reassign patients timely
Update agent unavailability in worker console
Initiate infection control forms
Send Employee Infection Report to HRD
Serve as back up during lunch hour
Receive calls from field staff
Assist with weekly case conferences
Refer clinical questions to Branch Director
Maintain client hospitalization log
Enter coordination notes
Send electronic log to staff
Complete requested schedule as task appears
Ensure staff are scheduled for visits
Schedule discharge visit / OASIS Collection
Schedule TIF OASIS collection visits
Delete remaining schedule
Reschedule declined or missed visits
Process reassigned and rescheduled visits
Ensure supervisory visits are scheduled
Run all scheduling reports
Prepare weekly Agent Schedules
Perform initial review of weekly schedule
Forward schedule to Branch Director
Verify visit paper notes
Assist with internal transfer of patients
Assess lab reports for normality
Fax lab copy to doctor
Make copy of lab for Case Manager
Route lab to Medical Records
Initiate Employee / Patient Infection Reports
Perform patient visits
Participate in on-call rotation
