Must Have
Licensed Professional Nurse or Licensed Vocational Nurse, 1 year of home health experience, Valid state driver's license, Automobile liability insurance, Currently licensed in the State of employment

What You'll Do.
Schedule patient visits
Communicate with field staff
Communicate with patients
Communicate with physicians
Maintain care coordination
Maintain continuity of care
Assist with office management
Assist with staff management
Manage schedules for all patients
Edit schedule for sick agents
Reassign patients timely
Update agent unavailability
Initiate infection control forms
Send Employee Infection Report to HRD
Receive calls from field staff
Assist with weekly case conferences
Refer clinical questions to Branch Director
Maintain client hospitalization log
Enter coordination notes
Send electronic log to staff
Complete requested schedule
Schedule skilled nurse visits
Schedule injection visits
Schedule aide supervisory visits
Schedule discharge visit
Schedule OASIS Collection visit
Schedule recert visit
Schedule TIF OASIS collection visits
Delete remaining schedule
Reschedule declined visits
Reschedule missed visits
Process reassigned visits
Process rescheduled visits
Ensure supervisory visits are scheduled
Run scheduling reports
Prepare weekly Agent Schedules
Review weekly schedule for productivity
Review weekly schedule for geographic issues
Forward schedule to Branch Director
Verify visit paper notes
Assist with internal transfer of patients
Assess lab reports for normality
Fax lab reports to doctor
Make copy of lab report for Case Manager
Route lab reports to Medical Records
Initiate Employee Infection Reports
Initiate Patient Infection Reports
Perform patient visits
Participate in on-call rotation
