Varied (United States of America) **Department:** 100 - 846000 TG - Cancer Center **Position Description:** The Patient Navigator helps patients access the full continuum of cancer care from screening to detection, diagnosis, treatment, and resources, available beyond the completion of treatment. This position concurrently facilitates proactive patient care services to ensure that care is appropriate, timely, cost effective and achieves the desired outcome; including, but not limited to, psychosocial assessment, patient advocacy, discharge planning, resource management, crisis intervention, community education, and counseling, designed to overcome barriers to care and meet the varying psychosocial needs of patients and their families relating to a cancer diagnosis. Key Responsibilities: • Conduct psychosocial assessments for new cancer diagnoses and develop intervention plans with appropriate referrals. • Document patient needs, interventions, and discharge planning in compliance with hospital policies. • Provide supportive counseling, grief education, and facilitate patient/family access to community resources. • Communicate and collaborate with physicians, interdisciplinary team members, and community agencies to support patient care. • Act as a patient navigator by coordinating care, appointments, education, and resources to prevent delays in treatment or discharge. • Lead support groups, provide community education, and serve as a patient advocate throughout the continuum of care. Qualifications: 1\. Associate's or Bachelor's Degree in a healthcare-related field (e.g., Nursing, Allied Health, Social Work, or Public Health) required. 2\. Clinical experience in healthcare or social services settings is preferred. 3\. Strong communication, assessment, and care coordination skills; knowledge of healthcare systems and resources. This position is seeking candidates with a background in Social Work or candidates holding a degree in Social Work.
