High School diploma or G. E. D., Meet Center for Medicaid/Medicare Services (CMS)-approved state and/or national certification requirements, All appropriate state licensure, education, and training, Demonstrated commitment to organization culture, values, and customer service standards, Successful completion of CPR certification

Previous patient care experience in a hospital setting or a related facility

What You'll Do.

Provide safe and effective dialysis therapy

Setup and operation of hemodialysis machines

Assist in maintenance of safe and clean environment

Support Quality Enhancement Program (QEP)

Participate in process improvement activities

Provide patient specific education

Welcome assigned patients

Evaluate vascular access for patency

Perform vascular access cannulation

Perform administration of heparin

Obtain pre and post treatment vital signs

Perform vascular access evaluation

Initiate dialysis treatment

Evaluate intradialytic problems

Monitor patients’ response to dialysis therapy

Obtain vital signs prior to discontinuation

Discontinue dialysis treatment

Evaluate patient prior to discharge

Perform and record pre and post dialysis evaluation

Calculate and entry of dialysis machine programming

Ensure appropriate safety alarms are enabled

Record accurate and timely information

Document information related to patient treatment

Report significant information

Observe patient and conduct machine safety checks

Perform and document intervention for unusual patient status

Evaluate patients’ vascular access during treatment

Obtain all prescribed laboratory testing

Prepare specimens for collection

Monitors patients performing self-care

Assists in training of direct patient care staff

Follow all organization business policies

Maintain environmental integrity and aesthetics

Clean and disinfect dialysis machine

Conduct tasks for preparation for dialysis treatment

Perform required pretreatment dialysis machine alarm testing

Initiate Solution Delivery System (SDS) system

Complete and document CQI activities

Enters all treatment data

Review treatment sheets for completeness

Prepare initial patient chart for admission

Compile paperwork for chart placement

Prepare/print lab requisitions

prepare and store lab samples

Ensure collection of lab specimens

Assists in collecting information for infection control audits

Supports staff and patient adherence to infection control practices

Follows infection control Policies and Procedures

Participates in and reinforces infection control education

Follows all newly admitted patients through their first 4 weeks

Coordinating their transitional dialysis care

Reinforces all education and care related matters

Liaisons with appropriate FKC staff members

Assists in assuring patient understanding of home dialysis products

Sets and preforms a treatment on all machines

Assist in assuring patient understanding of treatment options

Assists with coordination of transition to patient’s modality choice

Assists with supply ordering and inventory

Team & Collaboration

Assist other health care members in providing patient education; Report any significant information and/or change in patient condition directly to the registered nurse or supervisor; Report any change or unusual findings to the registered nurse or supervisor; Liaisons with appropriate FKC staff members to provide the best patient experience
