What You'll Do.
Provide safe and effective home dialysis therapy
Provide training for patients
Assist in setup of home dialysis equipment
Maintain safe and clean working environment
Support Quality Enhancement Program
Participate in process improvement activities
Reinforce home hemodialysis training
Reinforce peritoneal dialysis training
Identify and welcome patients
Gather information for Clinic Visit Intake form
Build communication processes with patients
Build rapport with patients
Maintain communication with care partners
Maintain communication with HT RN

Obtain treatment records from patients
