Eligibility Requirements

Must be able to take the Ishihara's Color Blindness test, Frequent, prolonged periods of standing, Bend over, Push and/or pull equipment, exerting up to 15 pounds of force, Lift and lower solutions up to 30 lbs, Occasional lift up to 40 lbs, as high as 5 feet, May be exposed to infectious and contagious diseases/materials, Direct patient care that regularly involves heavy lifting and moving of patients and assisting with ambulation

Previous patient care experience in a hospital setting or a related facility, Successful completion of the organization dialysis training program

What You'll Do.

Provide safe and effective dialysis therapy

Assist in providing patients a solid foundation

Care for and share information with patients

Setup and operation of hemodialysis machines

Maintenance of a safe and clean working environment

Assist in providing patient specific education

Welcome assigned patients and inquire wellbeing

Evaluate vascular access for patency

Perform vascular access cannulation

Administer heparin as delegated

Obtain pre and post treatment vital signs

Initiate dialysis treatment

Evaluate intradialytic problems and provide intervention

Monitor patients’ response to dialysis therapy

Discontinue dialysis treatment

Obtain Hemostasis and apply appropriate dressings

Evaluate the patient prior to discharge

Calculate and entry of dialysis machine programming

Ensure appropriate safety alarms are enabled

Record accurate and timely information

Report significant information to registered nurse

Observe patient and conduct machine safety checks

Perform and document intervention for unusual patient status

Evaluate patients’ vascular access during treatment

Obtain all prescribed laboratory testing

Clean blood spills with appropriate disinfectant

Monitor patients performing self-care

Assist in training of direct patient care staff

Follow all organization business policies

Maintain environmental integrity and aesthetics

Clean and disinfect dialysis machine

Conduct tasks for preparation for dialysis treatment

Initiate Solution Delivery System (SDS) system

Complete and document CQI activities

Enter all treatment data into clinical application

Review treatment sheets for completeness

Prepare initial patient chart for admission

Prepare/print lab requisitions

prepare and store lab samples

Ensure collection of lab specimens

Assist in collecting information for infection control audits

Support staff and patient adherence to infection control practices

Follow infection control Policies and Procedures

Reinforce infection control education of patients

Follow newly admitted patients through first 4 weeks

Coordinate transitional dialysis care

Reinforce education and care related matters

Liaise with appropriate FKC staff members

Assist in assuring patient understanding of home dialysis products

Set and perform treatment on all machines

Assist in assuring patient understanding of treatment options

Assist with coordination of transition to patient’s modality choice

Assist with supply ordering and inventory for TCU

Full Job Description

**_Transitional Care Unit (TCU) - Patient Care Technician_** A Transitional Care Unit (TCU) helps ease patients into dialysis. The first weeks of dialysis involve a great deal of adjustment, along with increased risk of hospitalization and mortality. As a TCU Patient Care Technician (PCT), you will assist in providing patients with a solid foundation to start their dialysis journey. The TCU PCT works in coordination with the TCU nurse to care for and share information with patients to help them feel better and make more informed decisions about their care, including choice of treatment modality—in-center or at-home dialysis. # _PURPOSE AND SCOPE:_ Functions as part of the dialysis health care team in providing safe and effective dialysis therapy for patients under the direct supervision of a licensed nurse in accordance with organization policies, procedures, and training and in compliance with regulations set forth by the corporation, state, and federal agencies. Responsible for the setup and operation of hemodialysis machines. Assist in the maintenance of a safe and clean working environment. Supports the organization commitment to the Quality Enhancement Program (QEP) and CQI Activities, including those related to patient satisfaction and actively participates in process improvement activities that enhance the likelihood that patients will achieve the organization Quality Enhancement Goals (QEP). # _PRINCIPAL DUTIES AND RESPONSIBILITIES:_ **_Patient Related_** _Education:_ * Assist other health care members in providing patient specific detailed education regarding adequacy measures where appropriate - Online Clearance Monitoring (OLC), Adequacy Monitoring Program (AMP), Urea Kinetic Modeling (UKM), and regarding disease process/access. _Treatment:_ * Welcome assigned patients and inquire as to their wellbeing since their last treatment. * Evaluate vascular access for patency, perform vascular access cannulation, and perform administration of heparin as delegated
