Must Have
High School diploma or G. E. D., CMS-approved state/national certification, CPR certification

What You'll Do.
Reinforce home dialysis training
Obtain vital signs and weights
Gather information for intake
Build communication with patients
Maintain rapport with team
Obtain treatment records
Report missing data to RN
Assist with medication reconciliation
Assist HT RNs with home visits
Prepare lab testing supplies
Prepare specimens for shipping
Track transplant specimens
Reinforce transplant education
Complete daily chores
Disinfect dialysis machines
Prepare exam/training room
Stock rooms with supplies
Schedule patient appointments
Contact patients for reminders
Support connected health use
Assist patients with ordering
Maintain logs and tracking
Order clinic/office supplies
Participate in audit processes
Collaborate during staff huddles
Collect admission documentation
Maintain patient chart
Perform clerical duties
Tear down HHD machine
Troubleshoot HHD machinery
Troubleshoot connectivity issues
Prepare dialysis supplies
Evaluate dialysis access
Document findings in record
Report abnormal findings to RN
Initiate HHD treatment
Monitor patient status
Document machine data
Perform safety checks
Report patient status changes
Provide interventions during treatment
Obtain pre-treatment vitals
Achieve hemostasis of access
Collect post-treatment vitals
Assist with water testing
Assist with dialysate testing
Reinforce PD training
Troubleshoot PD cycler issues
